Appointments can be made up to 24 hours in advance on line via appointments@perrymed.com or by calling 718.231.6700 (Our Extensions: Primary ext. 2, Specialty ext. 3, Lab ext. 4). Please note that changes or cancellations must be made 24 hours in advance. There is a $20 fee for missed appointments. Prior to your first appointment please download and fill out a registration form. Please bring the form with you to PAFM.
